How can I use this report?
- Understand the measures of quality diabetes care. You and your doctor can help each other stay on track for the recommended tests, exams and vaccines ? the "processes" that are measured below. Use this checklist to remind you of important questions to ask. Keep a record of your results.
- Know your goals for these "outcomes" measures of good control, and track of your progress in reaching them. Talk to your doctor if you have trouble with your medicines, or if you need help to lose weight or quit smoking.
- Ask questions about things you don't understand and about things you need help with. It's OK to ask questions!
