Improving Heart Failure Care in Greater Cleveland

How are doctors and patients in Greater Cleveland doing?

Pretty well, actually, and slightly better than our last report.

This is our second Community Health Checkup report on the care of patients with heart failure, and it describes care in 2009.  Over 275 doctors in 32 primary care practices at Cleveland Clinic, MetroHealth and Kaiser Permanente volunteer to measure and report on the evaluation and treatment of nearly 5,200 patients. 

According to our heart failure measures, in 2009, just under four out of five (79%) of our heart failure patients had received an appropriate evaluation of their condition, and nearly all (96%) of our patients with serious or moderate heart failure (which affects about two of every five of our heart failure patients) received appropriate treatment with at least one key medication.  Both of these numbers are slightly improved from our last report - six months ago.

We are particularly interested in exploring differences between heart failure patients according to their insurance type, race/ethnicity, income or education.  To explore these results, use the sorting features next to the graphs at the bottom of the page.
