Diabetes Care and Outcomes in Greater Cleveland, 2010-11

How well are you and your doctor managing your health?

Region-wide, we're holding our gains, but there's still a lot to do.

Our eighth Community Health Checkup report on diabetes covers July 2010 - June 2011, and includes information on 48 primary care practices, and over 500 doctors, providing care to nearly 29,000 patients with diabetes at one of the eight Greater Cleveland health systems reporting their performance to Better Health.

In 2010-11, according to our diabetes measures, just under half (47%) of our diabetes patients got the care they needed.  Also, just over two out of five (41%) diabetes patients in 2010-11 were in control of their key outcomes. 

We continue to see large differences between diabetes patients across different types of insurance, race/ethnicity, education, income, and by whether their health system has an electronic health record in place or is in transition from paper to electronic records.  To explore these results, use the sorting features next to the graphs at the bottom of the page.
